Once you’re ready to move forward, we’ll schedule a full assessment to guide your personalized care plan. This evaluation includes:
Medical Evaluation – To identify any physical health issues.
Psychiatric Assessment – To evaluate any co-occurring mental health conditions.
Substance Use History – To understand the scope of addiction and previous treatment efforts.
Family & Social Support Review – To explore your support network and how we can involve your loved ones in the healing process.
After your assessment, our clinical team will develop a personalized care plan just for you. This may include one or more of our evidence-based programs:
Partial Hospitalization Program (PHP)
Intensive Outpatient Program (IOP)
Outpatient Program (OP)
